New and Returning Students:

The following forms must be printed and signed:
(Click the title of the form to download and print. These are also available at the CHS band hall.)

- Physical Examination Form (Required to participate in marching band.)
- UIL Acknowledgement Form  (Freshmen only)


Band Fees and Required Items Price Sheets:
(We will have copies of these at registration, or you can print them at home.)
Registration takes place in August. This is when you will pay your band fees and purchase the items needed for each band member.

***Payment by Credit Card, Check, or Money Order will be accepted. Full payment is expected at time of registration. These forms include the prices for all fees/uniform items for the school year. The style and design of our uniform items stay consistent from year to year to minimize the cost to our students.
