If you already have a honor code, UIL acknowledgement , and a current medical release form,
you do not need to fill out a new form each year. The only forms that are needed annually are the band handbook signature form and the Participation Physical Examination Form.

Please join our Booster Club! Our Booster Club is an integral part of the CHS Band Program. We can always use your help and area of expertise.
7. Band Registration Order Form (form will be provided at registration)
Payment by Credit Card, Check, Money Order will be accepted. Full payment is expected at time of registration. These forms include the prices for all fees/uniforms items for the school year. The style and design of our uniform items stay consistent from year to year to minimize the cost to our students.

8. Band Plaque Receipt
If a business asks for a receipt when selling plaques, you may give them this document. Remember to save the bottom portion and turn in to a director.
